Supporting your child’s mental health starts with an intake assessment. Sign into MyChart to schedule an appointment. If you don’t have an account with us, you can easily create one or sign in as a guest. Please note this initial assessment is free.
Our program focuses on children and teenagers ages 8 through 17. At this time, we do not offer programs designed for patients who are developmentally delayed, have an active eating disorder, or have autism.
Assessment results help us determine which of our intensive outpatient programs best fits your child’s needs.
A parent/guardian should plan to be present during the assessment. Your input is valuable to our therapists, helping us to more accurately evaluate your child’s needs.
Sessions are conducted via video visit through the MyChart app. Please refrain from driving during your video chat appointment, as your safety is always our top priority.
Assessments can take between 1 hour and 1.5 hours to complete, so please plan accordingly.

You will need to e-check in 72 hours (three days) before your appointment to complete a short questionnaire and consent forms. Please do not wait until the day of the assessment to complete your check-in. Providing this information as soon as possible gives our intake assessment therapists time to review and get back to you with questions.
Copies of any relevant psychology testing or test results, including the most recent IEP or 504 plan from your child’s school
Custody documents
Complete list of all current medications and dosages
Completed patient history form - Pediatric - English/Spanish
Name of and contact information for your child’s referring physician, primary care physician, psychiatrist, and therapist
The first assessment is free. Once your child is placed in an outpatient program, you’ll need to contact your insurance provider directly to ensure coverage. We accept most insurances, including Aetna, Anthem Blue Shield, Blue Cross, Cigna, Exclusive Care, Healthnet, Managed Health Network, IEHP, Tricare, and Triwest.
