Our transplant team, including physicians and nurses, are available to you 24 hours a day, 365 days a year. Our goal is to support you and your family through the entire transplant process and beyond.

Your Patient Journey

We take a whole person approach to providing transplant care. Our team provide personal and individualized care to the patients during each phase of transplantation.

You will receive educational materials on transplantation during the different phases of the transplant process. Educational classes are also offered pre-transplant and during your hospitalization. The educational process continues through the post-transplant phase in our transplant clinic where we will monitor and support your progress.

Support Groups

We offer support groups for patients, families, friends and caregivers as part of our whole person care approach. During meetings, patients share their experiences before, during and after transplantation. These support groups are offered in both English and Spanish.

Case Management

Social work services are also provided to you and your family for support during the transplantation process.

You will be assigned to a case manager to ensure you are taken care of throughout the entire transplantation process. Coordinators work with you to provide personal and individualized care. They will help address psychosocial issues, which may adversely affect medical treatment or compliance with the medical regimen. Social workers contribute to treatment success by helping you cope with the therapy process.

  • Counseling (on-site within transplant clinic with marriage and family therapists)

  • Discharge planning

Child Life Specialist

Our child life specialists take an active role with our transplant children and their siblings. They offer the following services/events:

  • Grief Group
  • Grief Camp
  • Teen Group
  • Diabetic Support Group

Transplant Pharmacist

We have pharmacists that are specially trained in transplants and work closely with all of our transplant patients in overseeing their medications. They oversee all immunosuppressive and medications to ensure they are taken correctly to give you the support you need for a successful outcome.
