MTM, also called pharmacotherapy, involves the fine-tuning of medications to create a treatment plan that’s right for you. By adding a pharmacist to your care team, we can better monitor and adjust your medications for the best possible results.

  • Proactive care: Our pharmacists work proactively with your provider to determine if MTM can help you. That means you don’t have to seek us out — we come to you.
  • One-on-one consultations: We work closely with you to ensure you understand your medications, explain how to take them properly, develop an effective treatment plan, and answer all your questions.
  • Personalized treatment plan: We’ll adjust your medication doses to fit your health and lifestyle needs. We work to maximize medication benefits while reducing any side effects.
  • Ongoing monitoring: Our pharmacists fill in the gap between office appointments with your provider, helping monitor your condition(s) more closely.

Is MTM Right for Me?

You may be a candidate for MTM if your care team determines it’s necessary to help manage your conditions. For example, if your test results continuously fall short of your treatment goal, your provider may refer you to our clinic.

Only established patients qualify for referral, meaning you have received care from one of our primary or specialty care providers in the last three years.

What to Expect

If your care team determines you are eligible for MTM, the first step is to meet one-on-one with a pharmacist. This initial appointment will resemble an in-office visit with a provider, where you’ll meet with a pharmacist to review your condition and start developing a treatment plan. Follow-up visits usually take place via video visit through MyChart or over the phone.

Services We Offer

Here’s what you can generally expect from your MTM experience:

  • Medication review: A pharmacist will review your medications and ensure you are taking them safely and effectively.
  • Medication management: After reviewing your current treatment plan, we’ll make any necessary modifications to your medications and closely monitor for further adjustments.
  • Drug titration: We use titration to fine-tune your medications. Titration involves starting with a lower dose of a medication and gradually increasing it over time to find the best possible outcome.
  • Remote patient monitoring (RPM): Take-home monitoring allows us to quickly identify changes in your conditions. By tracking your blood pressure, glucose levels, and weight at home, we can make timely adjustments to your medication.
  • Pharmacotherapy clinic visits: Our pharmacotherapy clinic is located in the Center for Health Promotion building, offering convenient access to in-person visits with your pharmacist as needed.
  • Patient education: Your pharmacist will help guide you in taking your medications correctly, taking any necessary precautions, and watching for potential side effects.
