Managing multiple medications for different conditions can be overwhelming, but Medication Therapy Management (MTM) can help make it easier. MTM, also called pharmacotherapy, involves the fine-tuning of medications to create a treatment plan that’s right for you. By adding a pharmacist to your care team, we can better monitor and adjust your medications for the best possible results.
Our goal is to minimize side effects, ensure you are only taking the medications you need, and guide you in taking them correctly. We're here to help improve the way your medication supports your well-being.
Our expert pharmacists are committed to helping you manage one or more of the following chronic conditions:
You may be a candidate for MTM if your care team determines it’s necessary to help manage your conditions. For example, if your test results continuously fall short of your treatment goal, your provider may refer you to our clinic.
Only established patients qualify for referral, meaning you have received care from one of our primary or specialty care providers in the last three years.
If your care team determines you are eligible for MTM, the first step is to meet one-on-one with a pharmacist. This initial appointment will resemble an in-office visit with a provider, where you’ll meet with a pharmacist to review your condition and start developing a treatment plan. Follow-up visits usually take place via video visit through MyChart or over the phone.
Here’s what you can generally expect from your MTM experience:
Our providers and pharmacists work together to proactively identify whether you can benefit from medication therapy management — no action is required from you to get started. However, if you think you might benefit from this clinic, don’t hesitate to ask your provider if you’re eligible.
