Patient Transfer Center

Our dedicated Transfer Center team is available 24 hours a day to help you seamlessly transfer or refer a patient to our Medical Center, Children's Hospital, Surgical Hospital, or East Campus location. Your one-step request is coordinated by transfer center experts ready to get your patient the transfer care they need as efficiently as possible.

Loma Linda University Health serves a vast and diverse region, offering world-class care and Level I Trauma Center services. We accept patients from near and far for emergent care, urgent care, transplant, and a broad range of specialty and subspecialty services.

How to Request a Patient Transfer

Emergent Transfer

Call us at 800-865-5862 to initiate transfer of a patient requiring emergent higher level of care (HLOC).

All Other HLOC Transfers

For all other higher level of care transfer requests, please choose one of the following options:

• Fill out and fax our transfer request form to 909-558-0288. For all adult non-emergent requests, please also fill out and fax our Interfacility Transfer Communication Comprehensive form.
