What to Bring

  • Before your appointment, please fax your external medical records / reports related to your diagnosis to us at 909-558-2404. Without these, your appointment may have to be rescheduled.
  • Any radiology images (on CD or film).
  • Your ID, insurance card, and a list of any current medications along with their dosages.
